OK, I see the screenshot now. Looks like you may need to customize the output a bit–although it's not clear why that break is happening. Have you tried looking at it with Firebug to see what the HTML element is? (http://getfirebug.com)
If you post a link to the actual site, I may be able to take a closer look and say what is going on.
OfflineHmmm. What you sent me looks different than your screenshot. I see the fields all line up nicely in FF 13.
It seems that you're logged in when this is happening. Can you logout and see if the fields appear aligned or not? And once you're logged in, can you use Firebug to see which HTML element might be causing that? Since I don't have a login I can't see that.
Also, what browser are you using to view that?

OK, looks like your theme wants to add 1.8em to margin-left for the ul element. That's why you see the indentation.
So long term, I'm going to address this by making sure that all the BD field elements are handled uniformly (right now there is a mix of paragraph and unordered list elements, which is part of the issue). But to get you going now, the easiest thing to do would be to change the style of your theme to eliminate this 1.8em on the ul element for BD. Use the wpuf_customs class to qualify those ul elements.
Also, looking at your raw HTML, I see an unclosed ul element in there NOT coming from BD. That definitely points to a misbehaving theme. You might want to make sure your theme has valid HTML as well, since that could be the cause here.
