Nothing I’ve tried will fix these things. I’ve deleted the plugin entirely (including removing the folder via FTP from the plugins folder) and installed it again.
Hoping you have some suggestions.

I’m sorry to hear about this issue. What is your current version of BD plugin?
I don’t see any error message reported in your attached screenshots however, it seems like your regions hierarchy isn’t properly setup as all of the regions showing are top level regions. Can you please provide a little bit more details on what error message are you seeing and what exactly isn’t working in your regions module?
Generally, regions can be defined in a hierarchical fashion, for example: Continent->Country->State->County->City. You do not need to define all those levels if you don’t want to. By default, we pre-configure the Regions module with all Continents, the Countries of the world and the States of the United States. Additional regions and levels can be added easily to customize for your locale.

That’s part of what’s wrong… all of the cities are set with Alberta as the Parent.

From the moment I installed the plugin, I got the top error and the hierarchy error really is wrong because what it’s saying that I don’t have – I do.
Also in that screenshot, you can see only Alberta shows, yet it says there are 22 items, and page two is empty.
I just want the Province of Alberta and then a few cities available in my regions.
Also, when I click that link that says “Restore Region Form Fields” it creates a Continent and a Country field but the message doesn’t go away and if I click it again it creates duplicates.
Thanks for that information. Since you don’t have that many region added, I’d recommend you reinstall the Regions module and start fresh.
The default hierarchy is Continent->County->State->City so in your case, the best method is to just rename your Continent field to “state” and Country to “City” etc. and then remove the original state and city fields etc. In other words, instead of removing top level regions (for example, continent), it is safe to rename them per your needs and then remove the lower level regions if required. This is a cleaner way of managing your regions hierarchy if you do not want continent etc.
The important part is to understand which are parent regions and which are child regions. We give you the most common use of Regions module pre-configured, but you are free to change that (including the labels of the fields) however you like. This can be done from BD Admin -> Manage Form Fields section.

I did previously try removing the plugin completely – I even deleted the plugin folder for it on a second attempt. Both times when I reinstalled the plugin, all of the settings that I’d previously configured remained as I had them configured.
I also tried the restore to defaults button. And I had changed the regions and fields the way you described. But it was all muddled up.
I just reactivated the plugin once more (without deleting and reinstalling)… and poof, the gremlins have disappeared lol. It’s working now as expected. So weird. But I should be used to weird stuff like that, I’m a Happiness Engineer with WordPress – it happens all day every day LOL.
